The range of discounts members receive will vary by provider and the type of service received. 8. Discounts for pharmacies are only available at participating pharmacies. 9. Qualified Health is a private label marketer for the Capella Group, located at 4929 W. Royal Lane, Suite 200 Irving, TX 75063 and is the Discount Medical Plan Organization that administers this Plan. 10. The plan includes a membership in America's Health Care Consumer Association. Not available in all states. Membership in this association includes the benefits described and may also include certain insurance benefits depending on the plan that you have purchased. These benefits do not constitute a health insurance. Details about your membership in this association will be provided in your membership kit.

A health discount program is not considered a plan for purposes of creditable coverage.
A certificate creditable coverage will NOT be issued upon termination of the discount program.
