Hospital

The Qualified Health network includes over 360,000 licensed physicians nationwide. All providers have met strict licensing and medical care practice requirements. Providers are available in most areas of medicine, including: Surgery, Family Practice, Internal Medicine, Pediatrics, Sports Medicine, OB/GYN, Cardiology, Urology, Radiology, Mammography, Orthopedics, Radiology and many other specialties. On a routine doctor's visit, members save an average of 20-40% off the regular fee, and 30-60% on the lab and diagnostic work that usually follows a routine doctor visit, such as X-rays, mammograms, blood work, MRIs, CT scans. The bigger discounts can sometimes be seen on the expensive bills, where the savings are needed most.

Hearing
Qualified Health members can purchase over 75 models of major brand-name hearing aids at 15% below retail dealer prices. Members also save on repair services and batteries.
Vision
Qualified Health members do not pay full price for eyeglasses, contact lenses, non-prescription sunglasses, eye exams, surgical procedures and other items. Our comprehensive network provides savings of 20 to 60% at vision care centers, as well as ophthalmologists, optometrists and all eye care related facilities.
Prescription Drugs
Qualified Health members see great discounts when filling their prescriptions at the local pharmacy. With our huge network of participating pharmacies, there is a pharmacy in your area.
Over 58,000 participating pharmacies nationwide including Albertsons, A&P, Bi-Lo, Costco, CVS, Dominick's, Duane Reade, Eckerd, Fred's, Fry's, Hy-Vee, Kmart, Kroger, Publix, Rite-Aid, Safeway, Shop-Rite, Target, Wal-Mart, Walgreens and Winn-Dixie Pharmacies and many more. Discounts on Prescriptions are only available at participating pharmacies.
For the medications that are taken on a regular basis, members can save even more significantly using our mail-order prescription program. Qualified Health gives you access to a wide range of medications at the lowest prices available on most brand name medications! There are Customer Care Representatives available to assist you at any time, and they can even contact your physician directly to avoid you the hassle of faxing or mailing prescriptions yourself.
24-Hour Health Hotline
Qualified Health members can contact a Registered Nurse at any time, day or night to obtain assistance with health concerns or problems. This often helps avoid unnecessary trips to the doctor or emergency room. Nurses are on call to answer your questions about medical tests, diagnosis, medical procedures, poison control, burn treatments, newborn assistance, drug interactions, high fever and more.
Chiropractors & Alternative Medicine
Qualified Health members save at all participating chiropractors, massage therapists, acupuncturists and numerous other specialists. There is no limit to the number of visits and no limit to any specific type of care; visits may be for care which is necessary for medical reasons, or for preventative or maintenance.
Patient Advocacy
The Qualified Health Team helps members in with finding a participating provider, assisting with providers on acceptance of Program, assisting providers with contracted fee schedule for active members and other routine service issues regarding the Program.
Medical Supplies and Equipment
Qualified Health members save on durable medical equipment (DME's). All items can be shipped to your home.
Diabetes Care Plan
A Qualified Health Membership offers a comprehensive diabetes maintenance program to help you take charge and manage your chronic diabetes conditions and increase patient compliance for a “quality of life” enhancement. The program provides access to a full line of state-of-the-art glucose meters, test strips, lancets and oral diabetic medications.
Members can experience over 50% savings every month, compared to average retail prices, on diabetes maintenance supplies and oral diabetic medications. To find a location where you can purchase diabetes supplies simply call the toll-free Member Services number listed on your Membership Card.
Lifestyle Benefits
Additional Lifestyle benefits include:
$2,500 Accident Medical Expense Benefit**
This benefit pays part of the expenses you are charged by a hospital, doctor, or certain other charges, up to a maximum of the amount listed if you are injured in a covered accident. Subject to a $100 deductible.
$5,000 Accidental Death and Dismemberment**
You’re covered 24-hours a day. This benefit pays up to $5,000 for the primary member’s accidental death or dismemberment in a covered accident.
*If you select the Family Plan, the spouse will be covered up to 50% (60% if there are no dependent children) and your children will be covered for 20% (25% if there is no spouse). *This paragraph does not apply to New York State residents. * In Florida, each program includes a selection of benefits.
